BioDigit Cognition™
Digital cognitive assessments with automated analysis for clinic and home use.
BioDigit Cognition supports the administration and automated analysis of cognitive assessments in clinical research. Digital tasks such as verbal fluency, oral SDMT, and trail making provide quantitative measures of cognitive performance in the clinic and at home.
Preliminary evidence for automated verbal fluency scoring in PSP
Preliminary, unpublished analyses from BioDigit PSP2 found close agreement with blinded expert scoring (ICC 0.96 to 0.98) and a decline in categorical fluency over one year. These results are specific to the studied PSP cohort.
Measuring cognitive performance over time
Cognitive changes can affect people with neurological and mental health conditions. Repeated assessments help researchers characterize performance and explore how it changes over time. BioDigit Cognition combines standardized assessment procedures with automated analysis: it captures task-specific responses, timing, and accuracy to support longitudinal research across sites and remote settings. The suitability of each measure as a trial endpoint depends on its validation for the intended use.
What BioDigit Cognition includes
A configurable assessment suite covers processing speed, attention, working memory, executive function, and language.
-
Verbal fluency
Timed word generation tasks that assess verbal retrieval and aspects of executive function.
-
Oral SDMT
The oral Symbol Digit Modalities Test assesses processing speed and attention using spoken responses.
-
Trail making
A digital sequencing task that records completion time and accuracy; alternating sequences assess set shifting.
-
Forward and backward digit span
Recall of digit sequences in forward and reverse order to assess attention and working memory.
-
Conflicting instructions
A task that assesses executive control by requiring responses that conflict with a given cue.
-
Additional assessments
Go/no-go and other digital tasks selected and configured for the cognitive measures and procedures in each study protocol.
How automated analysis works
BioDigit Cognition uses automated speech recognition and task-specific algorithms to derive scores and timing measures from participant responses. Automated analysis supports consistent scoring and reduces manual work, with review procedures defined for each study.
-
Response and timing measures
Task-specific outputs include response accuracy, response times, and completion times.
-
Automated speech recognition
Speech recognition processes recorded verbal responses to support automated scoring of speech-based tasks.
-
Objective measures
Participant responses are converted into quantitative measures of performance on specific cognitive tasks.
-
Standardized scoring
Defined scoring rules reduce variability introduced by manual scoring; performance is evaluated for each task.
-
Clinic and home use
A shared analysis workflow supports clinic and home assessments, with administration procedures defined by the protocol.
-
Endpoint development
Quantitative measures support endpoint development and evaluation for specific trial populations and uses.
Evidence for automated cognitive measures
Preliminary, unpublished analyses from BioDigit PSP2 (NCT05956834) evaluated agreement between automated verbal fluency scores and blinded expert ratings, along with change over one year. These findings support further evaluation of the measures; they do not establish validity across all tasks, populations, or trial uses.
-
Agreement with expert scoring
In 41 participants with PSP, remotely collected categorical and phonemic fluency word counts agreed closely with blinded expert scores: r = 0.93 to 0.97 and ICC = 0.96 to 0.98. These preliminary results assess scoring agreement, not test-retest reliability.
-
Longitudinal fluency findings
In 30 participants assessed every three months, categorical fluency declined by an estimated 2.47 words per year after adjustment for task version. MoCA scores did not change significantly; this preliminary comparison does not establish superiority to MoCA.
$3M NIH award supports integration with the PSP Trial Platform
Supported by a $3 million NIH award, BioSensics is integrating its digital health technologies, including BioDigit Cognition, with the Progressive Supranuclear Palsy Trial Platform (PTP). The platform is designed to evaluate multiple candidate therapies under a shared protocol, with approximately 50 sites planned across North America.
